Imphal, May 16: Over 4000 forms for appearing in the MBBS/BDS entrance test for selection as State nominees during the academic session 2004 have been issued till yesterday, according to a source from State Health Department.The last date of submission of forms is May 21 and the entrance test would be held on May 26.Meanwhile, Director of Health Services Ch Chandamani Singh has notified to all the applicants who have collected forms for the MBBS/BDS entrance examination, 2004 without the computer scan form on May 10 to collect the same from the Medical Directorate, Imphal on production of the application form/receipt already taken on or before May 20 during office honour.

Submission of application forms without the computer scan form would be rejected, the notification said.
